HOME VISITS
Primary Care and Urgent Care at Home
Personalized Healthcare, Right Where You Are
Healthcare doesn’t always need to happen inside a traditional clinic.
At Rooted Health, we offer personalized home visits for established and appropriate patients who prefer to receive primary care in the comfort and privacy of their own home.
Whether getting to the clinic is difficult, you have a busy schedule, or you simply prefer a more personal approach to healthcare, a home visit allows us to slow down, listen, and look at the bigger picture.
Same thoughtful, root-cause-focused care. A different setting.
What Is a Rooted Health Home Visit?
A home visit is a scheduled primary care or urgent care appointment provided in your home rather than at the Rooted Health clinic.
Home visits are designed to provide the same individualized, relationship-centered care you would receive in the clinic, with the added convenience of being seen in a familiar environment.
Depending on your needs, a home visit may include:
Urgent Care needs including fever, cough, upper respiratory symptoms, sore throat, rash, wounds requiring sutures or staples, urinary symptoms and more.
A comprehensive health history and discussion of your current concerns
Review of medications and supplements
Vital signs and physical examination
Chronic disease management
Preventive healthcare
Home visits may be especially helpful when your home environment provides additional context that can help us better understand your health and daily routines.
Who May Benefit From a Home Visit?
Home visits may be a good option for patients who:
Have difficulty traveling to a clinic
Mobility limitations, transportation challenges, recovery from an illness or procedure, or other circumstances can make traditional office visits difficult.
Prefer the convenience of care at home
For some patients, avoiding traffic, waiting rooms, and rearranging an entire day around an appointment simply makes sense.
Want a more personal healthcare experience
Being in your own environment can make it easier to have an open conversation about your health, routines, medications, nutrition, sleep, and other factors that influence how you feel.
Need ongoing primary care
Home visits can be used for appropriate primary care needs, including preventive care and management of ongoing health concerns.
Want a whole-person approach
At Rooted Health, we look beyond a single symptom. We consider the many factors that contribute to health—including nutrition, movement, sleep, stress, medications, environment, and daily habits.

Why Receive Care at Home?
A More Comfortable Environment
For many people, healthcare feels different when they aren't sitting in a traditional exam room.
Being at home can create a more relaxed environment where you can ask questions, have a meaningful conversation, and focus on your health without feeling rushed.
Care That Fits Into Your Life
Your health is influenced by what happens outside the clinic.
Seeing you in your home can sometimes provide helpful insight into your routines, medications, nutrition, movement, sleep environment, and other factors that may be difficult to discuss or understand during a brief office visit.
More Personalized Attention
Rooted Health is intentionally designed around personalized, relationship-based primary care.
We take the time to understand your individual goals and create a plan that makes sense for you—not simply treat a diagnosis or symptom in isolation.
How Home Visits Work
1. Request a Home Visit
Contact Rooted Health to determine whether a home visit is appropriate for your needs.
Because home visits require additional scheduling and travel considerations, availability is limited.
2. Discuss Your Needs
Before your appointment, we'll discuss your concerns and determine what type of visit will be most appropriate.
3. Receive Care in Your Home
We'll come to your home at the scheduled time and provide your appointment in a comfortable, private setting.
4. Leave With a Personalized Plan
Your visit will end with a clear understanding of next steps, which may include laboratory testing, medications, lifestyle recommendations, referrals, or follow-up care.
Cash-Based Care, Without the Insurance Runaround
Rooted Health is a cash-based primary care practice, which means you pay directly for your care rather than having your visit billed through insurance.
This allows us to focus on your health and your goals without being restricted by traditional insurance-based appointment structures.
Home visits are subject to the applicable Rooted Health visit fee and any additional travel or service fees that apply.
Before scheduling, we'll clearly explain the expected cost so there are no surprises.
Can I Use My HSA or FSA?
Our system does accept HSA or FSA cards.
If you have questions about using your HSA or FSA for Rooted Health services, we can provide documentation for you to submit to your plan when appropriate. Ultimately, eligibility and reimbursement are determined by your individual plan.
What About Insurance?
Rooted Health does not bill insurance for home visits.
However, depending on your insurance plan and circumstances, you may be able to submit documentation for potential reimbursement.
If appropriate, Rooted Health can provide a superbill containing the information you may need to submit to your insurance company.
Your insurance company—not Rooted Health—determines whether any portion of your care is reimbursable and whether the expense applies toward your deductible.
We recommend contacting your insurance company before your visit if you are considering submitting a superbill.
Home Visits vs. Traditional Primary Care
Traditional primary care often has to operate within the constraints of insurance networks, short appointment times, and high patient volumes.
Rooted Health takes a different approach.

Your health. Your home. Your care.
